Description |
Main duties and responsibilities include responsibility for and contribution to the preparation of care plans, their updating
And review by the care team, relevant to each individual resident’s needs. This should be done in consultation with residents, advocates and their families.
To ensure effective, open and positive communication with working relationships in the Home between residents, staff, relatives, volunteers, other professionals and all visitors to the home, using the Key Working system, as appropriate. All to be recorded and documented as appropriate.
