Description |
|
Low Intensity Primary Care Case Worker (Low Intensity Worker) will provide high volume case management within a primary care psychological therapy service in Colchester and Tendring. The Low Intensity Worker will deliver a range of low intensity interventions within a stepped-care model, with a focus on brief psychological interventions and medications management, and also a range of minimally supported guided self-help interventions including bibliotherapy and cCBT, psycho-education groups and wellbeing support, and also helping patients to access other appropriate services across the area. The role includes working with other members of the primary healthcare team, including GPs and practice staff, using a patient case management information system (PC-MIS) and capturing Routine Outcome Measures for the service. Low Intensity Workers will work within a Neighbourhood Team, and also with co-workers in other services, including Support, Time & Recovery workers.
